RN Case Manager - Fast paced, fast growing, fun team seeking RN Case Manager who is passionate about providing quality skilled nursing care, focusing on symptom management and pain control for terminally ill individuals in accordance with the interdisciplinary plan of care. You'll work approximately 3-4 weekends in 3 month period, occasionally may carry pager and work nights to cover for regular on-call staff. Day position with occasional nights.

Qualifications:

• Registered Nurse currently licensed in the state of MN
• Previous experience in hospice, home health, geriatrics/long term care, oncology or general medical surgical nursing preferred
• Demonstrated commitment to the hospice philosophy of care
